Light vehicle carrying explosives hit by underground loader
What happened?
While reversing an underground loader hit a light vehicle carrying explosives. Recently there have been a number of incidents where underground vehicles have hit explosives transport vehicles. The explosives did not explode but they could be initiated through friction, impact or fire from a vehicle accident.
Recommendations
- Safety management systems include suitable and appropriate procedures and controls that minimize the risk of interaction between explosives transport vehicles and other site mobile equipment to an acceptable level of risk.
- Explosives transport vehicles be fitted with appropriate equipment that allows their ready identification and detection. This may include the reflective explosives signage, operational flashing light that warns other vehicles and pedestrians, effective communication systems or proximity detection equipment.
- Safety management systems are reviewed to ensure suitable controls and procedures are in place to address the recommendations.
- Everyone involved in these activities follows the safety management system, controls and standard operating procedures.
